Who should I contact if I haven't been paid in a month and still not have rec'd my T4 slip?

I am a professional in the janitorial/maintenance field. I started working for a commercial cleaning company in Newmarket, Ont. last Sept. 2005. I was paid by the hour, used my own vehicle to haul cleaning equip around to multple clients, and was never offered gas/maintenance allowance. Right after New Years, fellow employees and myself were put on weekly salary ($11/hr for a 40 hr week.) without any of us agreeing to it, and some of us put in 50 to 60 hrs wkly! Then we started getting notes on hour pay chqs asking us not to cash our chqs until the following week. I put up with it until I started getting no pay chqs at all and have since then walked. It's now been a couple of months and no one is returning my calls, no one has been in the office when I've showed up in person although they are still operational., and they still owe me pay and my T4 slip. I've emailed the Ontario Labour Board with this same question and have rec'd no reply from them either. Who do I contact?
